Lost or Stolen Passport Replacement
If your passport is lost or stolen, you are advised to report the loss/theft to the local police in the area in which it occurred. In order to replace the passport, it will be necessary for you to appear personally at the Consulate. You should submit your application for a replacement passport as soon as possible after the loss or theft is discovered.
Application Requirements
Please bring the following with you:
- Proof of U.S. citizenship:
- a U.S. birth certificate;
- naturalization certificate;
- certificate of citizenship;
- consular report of birth abroad; or
- an expired passport;
- Proof of identity
- a valid passport of any country;
- a valid driver's license with photograph;
- a national identity card with photograph;
- a valid U.S. or state government ID card;
- a U.S. military ID card; or
- Identifying Witness: If you are unable to present photo identification as specified above, you should bring a relative, friend or traveling companion (preferably a family member) who has known you for over two years and who can present a valid, preferably U.S., passport, and who is able to submit a sworn statement confirming your identity.
- Passport application form DS-11: Form DS-11 must be printed on separate sheets of paper; do NOT print this form on front and back of the same sheet. For tips on completing the DS-11, follow this link;
- Passport form DS-64: Statement Regarding Lost or Stolen Passport
- Police report of the lost or stolen passport.
- Two identical passport photographs: The consular section will reject any photographs that do not meet the specified requirements. Vending machine photographs and photographs printed on a home computer are unacceptable;
- Fees: The application fee is U.S. $100.00 or the equivalent in Nafl. Accepted payment methods are:
- A self-addressed, prepaid commercial courier waybill to have your documents returned to you once processing is completed. Alternatively, you can pay for the courier service the day you visit the consulate.
Method of Application
You are required to apply for the passport in person.
Processing Time
An application for a full-validity (five years for children up to age 16, ten years for adults) passport will be sent to the United States for printing with a secure, photo-digitized image of the bearer. You should allow 15 working days for your application to be processed.
If you are traveling within 15 working days and can provide evidence of this travel, you may be eligible for an emergency limited-validity passport.
